The differentiation of good, evil, favorable and unfavorable is a method of differentiation of syndromes for prognosis of the surgical diseases. In the surgical diseases, "Good" means good phenomena; "Evil" implies bad signs. The differentiation of syndromes upon the ideas of the good and bad is usually to make a judgment from the general symptoms and signs. "Favorable" indicates the signs conforming to the rules of the normal development in the surgical diseases "Unfavorable" indicates the signs deviating from the rules of the normal development in the surgical diseases. The differentiation of favourable and unfavourable is mainly applied for differentiation of the local symptoms.

Good Signs of the Heart
Freshness of mind, fluent speech, moist tongue without thirst, peaceful sleep.
Good Signs of the Liver
Lightness of body, peaceful mind, rosy and lustrous nails, smooth urination and defecation.

Moist lips, normal taste, yellow and tenacious pus, normal defecation.

Sonorous voice, no wheezing and cough, even breathe, moist and lustrous skin.

Favorable Signs
Yang Syndromes
Initial the ulcer enlarges progressively in size, with elevated tumefaction in shrunken base, red color and burning pain.
Formed Localized red tumefaction, with bright and thin skin, and soft top engulfing the finger under pressure.
Ulcerous Pus Thick and tenacious pus in yellow and white color, consequently relief of pain and tumefaction.
Healing Exfoliation of ulcerated pus, fresh and red granulation in the wound, no obvious swelling and pain, gradual healing in the wound.
Yin Syndromes
Initial No obvious tumefaction, slight pain, no warm or slightly warm sensation in the local area, no change in skin color or slight pale color in the skin.
Formed Gradual appearance of tumefaction, slightly red color and slightly warm sensation in the local region, more obvious pain than before, fluctuation sense in the fingers by palpation.
Ulcerous Pus Discharged pus in grey and white color and thin in nature, gradual relief in the local symptoms.
Healing Gradual disappearance of pus, slightly red color in the wound, gradual healing in the wound.

Bad Signs of the Heart
Confused mind, vexation, dry tongue, purple and dark ulcer, twittering speech.
Bad Signs of the Liver
Stiffness in the body, difficulty in looking straight forward, bleeding in ulcer, frequent palpitation.
Bad Signs of the Spleen
Emaciation, depressed ulcer with fishy pus, no appetite, vomiting after taking drug.

Dry and coarse skin, profuse sputum and mute voice,gasping breath, flaring of nasal wings.

Unfavourable Signs
Yang Syndromes
Initial Flat ulcer with diffuse base, no red color and heat sensation, slight pain.
Formed Depressed ulcer with soft top, no severe pain, purple dark color.
Ulcerous Pus Scanty discharged pus, in thin nature, or mixed with blood fluid, with peripheral tumefaction and serious pain.
Healing Lingering pus, not fresh in the surface of the wound, or tightly attached with a layer of pus, with long duration to heal the wound.
Yin Syndromes
Initial Diffuse ulcer form, in a large scope, obvious pain at night, swift aggravation of the pathological change.
Formed Lingering tumefaction, soft and depressed ulcer on the top, no change in the skin color, or purple dark color in the skin.
Ulcerous Pus Large discharged volume of pus in thin nature, and grey and dim luster, mixed with
substance like cotton waste, or bleeding fluid, and swelling and lingering pain.
Healing Lingering pus and fluid, depressed border of the ulcer, empty cavity underneath the skin,
grey and white color in the surface of the ulcer, not fresh in granulation, or noticeable sinus, difficulty to heal the wound.
In the process of the diseases, good and evil, favourable and unfavourable are all relative. With the change of strength between the anti-pathogenic ability and pathogenic conditions, and the therapeutic and nursing measures, some changes may take place. Therefore, in the differentiation of syndromes of the surgical diseases, when the pathological changes are manifested in good signs and favourable signs, it is necessary to seize the opportunity for treatment and prevent any reversion carefully. When the diseases are manifested by bad signs and unfavourable signs, in addition to the sufficient attention, it is requested not to lose confidence and it is necessary to treat the diseases actively and nurse the patients carefully, in order to reverse the symptoms to good signs and favourable signs as early as possible.
