As mentioned above, normal urine is, in general, without scum or precipitation.Occasional precipitation might be seen after long standing, which is almost invisible. Once floating materials and precipitation are readily seen, it indicates disease. Usually, the floating material in the urine is flocculent. Its color, form, location should be carefully examined. It varies in different diseases.

Rlung disorders: the floccules look like goat's wool that can't be taken out when touched with a small rod.
Mkhris pa disorders: The floccules look like a cotton ball, with a dense center and loose periphery. It can be precipitated to the bottom.

Badkan disorders: the floccules look like the caudal hairs of a horse, without distinct demarcation with the urine around it.
Lung heat: the floating materials are in the form of a moving white cloud, With Pus formation: all internal diseases with abscess may reveal floating material in the form of pus.

Kidney diseases: the floating material in the urine is in the form of fine sand that can be precipitated to the bottom. Its location in the urine should also be examined. This is useful to determine the location of the diseased. Generally speaking, its location in the urine is of three kinds, the upper, the middle and the lower layers. Upper urine floating material indicates the diseased portion is above the diaphragm, i. e. diseases of the heart or lung. Floating material located in the middle layer of the urine indicates a disease located in the upper abdomen, or in the liver, spleen, gall bladder, and diaphragm.

Precipitation at the bottom part indicates that the disease location is in the lower abdomen including the pelvis, or diseases of the kidney, bladder, small intestine and genital organs.

Scum
It refers to a thin membrane on the surface of the urine, which should be examined after the urine has stood for a while. Generally, scum, if any, appears after the urine cools down.

The thickness of scum: A thin scum indicates cold disease; whereas a thick one shows hot disease. When it is allowed to stand for a certain time and the scum breaks into fragments, it is a lump disease. There is a kind of thicker white scum, which, when removed with a small rod and placed on the nail, will not break. Put the scum on the fire and a smell of baked meat will be emitted. This is due to overeating of fat, and more fatty membrane is excreted. This is not a pathological condition and needn't be treated.
