Differentiation and Treatment of Common Syndromes m Traditional Chinese Medicine

1. Diarrhea due to Damp-heat Pathogen (Occuring in acute diarrhea) :

Clinical manifestations: Abdominal pain, diarrhea with bloody mucous stool which is moved several times or more than ten times a day with a small amount each time, tenesmus, burning sensation at the anus, scanty deep-coloured urine, or accompanied with chill and fever, yellow and greasy coating of the tongue, slippery and rapid pulse.

Therapeutic method: Clearing away heat and eliminating dampness and regulating qi to remove stagnancy.

Recipe: Modified Peony Decoction.

Ingredients:

Radix Paeoniae Alba
Rhizoma Coptidis
Radix Scutellariae
Flos Lonicerae
Herba Portulacae
Radix et Rhizoma Rhei
Radix Aucklandiae
Semen Arecae
Fructus Aurantii
Radix Glycyrrhizae

Administration: All the above drugs are to be decocted in water to get 200-300ml of decoction. Take equal portions in the morning and in the evening.

Modification: For the case with the onset of chill an fever, Rhizoma Puerariae 15g is usually added.

2. Fulminant diarrhea (mostly occuring in toxic diarrhea) :

Clinical manifestations: Sudden onset, violent attack, pu-rulent and bloody stools, or bloody stools, high fever, restless-ness or even coma or convulsion in serious cases, red tongue or deep-red tongue with yellow greasy fur and rapid pulse.

Therapeutic method: Removing heat and toxic materials and cooling the blood.

Recipe: Modified Pulsatilla Decoction.

Ingredients:

Radix Pulsatillae
Cortex Fraxini
Rhizoma Coptidis
Cortex Phellodendri
Flos Lonicerae
Fructus Forsythiae
Cortex Moutan Radicis
Radix Paeoniae Rubra
Radix Sanguisorbae
Radix Puerariae

Administration: All the above drugs are to be decocted in water to get 200--300ml of decoction. Take equal portions in the morning and in the evening.

Modification: If there appears coma, convulsion or toxic shock, Bezoar Sedative Bolus, Purple Snowy Powder and the like may be added. Meanwhile, the treatment of traditional Chinese medicine should be given in combination with that of Western medicine.

3. Recurrent diarrhea (mostly found in chronic diarrhea) :

Clinical manifestations: Chronic diarrhea with frequent relapse, and slow recovery, abdominal pain and tenesmus dur-ing the attack, dark-coloured mucous stools, fatigue and weakness, pale tongue with greasy fur, deep and weak pulse.

Therapeutic method: Invigorating the spleen, replenish-ing qi, removing heat and eliminating dampness.

Recipe: Modified Decoction of Four Noble Drugs.

Radix Pulsatillae
Radix Sanguisorbae

In case of diarrhea of deficiency-cold type with symptoms of watery and purulent stools, abdominal pain with preference for warmth and pressure, cold extremities, loss of appetite, listlessness, pale tongue with whitish fur, deep and fine pulse, it is preferable to employ Modified Decoction {or Nourishing the Viscera, including:
